I hereby authorize the staff of American Karate Studios, Inc. to act for me according to their best judgment in any emergency requiring medical attention and I hereby waive and release American Karate Studios, Inc. from any and all liability for any injuries or illness incurred while at Camp.  I have no knowledge of any physical impairment that would be affected by the above name Camper’s participation in the Camp program.  I also understand that American Karate Studios, Inc. retains the right to use for publicity and advertising purposes, photographs and videotapes of Campers taken at Camp.

REGISTRATION

A deposit of $50 per week of camp must accompany the application, with the balance to be paid in full by June 1, 2010.  Upon receipt of your application, you will receive a confirmation, information form, Karate Kamp Rules and Regulations, and detailed information regarding camp.  Full payment of the tuition is required for all applications received after June 1, 2010.  Call or stop in to register or print and mail in our web application.  Class size limited so ENROLL EARLY! School T-Shirt required at $12.00 per shirt.  Uniforms optional for $30.00.

REFUNDS

Full refunds will be issued prior to May 1, 2010.  After May 1, 2010, with receipt of written cancellation notice, refunds, minus a $50.00 administrative fee, will be issued in the form of a tuition credit that can be used towards any future American Karate Studios, Inc. activities.  We reserve the right to dismiss any camper whose conduct is detrimental to the camp and no refund will be issued.  No deduction is made for late arrival or early departure.  No refund will be made for withdrawal or absence due to illness or family vacation.

CAMP HOURS

Camp hours are from 9 am to 3:30 pm for full day sessions.  Half day am session hours are from 9 am to 12 pm and half day pm session hours are from 12:30 pm to 3:30 pm.

EXTENDED HOURS

Extended hours are available for working parents for an additional $7 per hour.  Please contact school office to make arrangements.

SNACKS AND LUNCH

Lunch and snacks are not provided except for Special Activity Days.

CANCELLATION

American Karate reserves the right to cancel courses for inadequate enrollment.  If requested sessions are booked, American Karate Studios, Inc. will make every effort to accommodate your child in another sessions or time slot.

TRIAL CLASS

A free trial class may be scheduled prior to enrollment before 5/1/11, by appointment.  Call for more information or stop in to view our facilities.
